3. Services Provided
Mindful Care Hubs provides licensed medical services including but not limited to General Healthcare, Home Nursing Care, Doctor Home Visits, Diabetes Management, Orthopedic Care, and Dental Care. All services are rendered by qualified, licensed professionals. We reserve the right to decline services if a patient's condition requires hospital-level care.
4. Appointment Booking & Cancellations
Appointments must be booked through our website, phone, or authorized representatives. Cancellations or rescheduling must be made at least 6 hours in advance. Late cancellations may be subject to a service fee of up to 50% of the scheduled visit cost.
5. Payment Terms
Fees are disclosed prior to service confirmation. We accept major credit/debit cards, ACH transfers, and approved insurance plans. Payment is due at the time of service unless pre-arranged. Outstanding balances accrue 1.5% monthly interest.
6. Patient Responsibilities
You agree to:
- Provide accurate medical history and current medication details.
- Ensure a safe, accessible environment for home visits.
- Inform us of any changes in health condition promptly.
- Treat staff with respect; we reserve the right to refuse service for abusive conduct.
7. Limitation of Liability
While we adhere to the highest clinical standards, healthcare involves inherent risks. Mindful Care Hubs shall not be liable for indirect, incidental, or consequential damages. Our total liability is limited to the fees paid for the specific service in dispute.
